Apolipoprotein B (mg/dL)
Apolipoprotein B (ApoB) measures the total number of atherogenic lipoprotein particles in your blood, since each LDL, VLDL, and Lp(a) particle carries exactly one ApoB protein. People test it because particle count is increasingly viewed as a more direct gauge of cardiovascular risk exposure than LDL cholesterol alone.

The importance of ApoB
Cholesterol travels in particles, and it is the particles, not the cholesterol itself, that enter and lodge in artery walls. Because every atherogenic particle carries one ApoB molecule, ApoB is effectively a headcount of those particles. Two people with identical LDL-C can have very different ApoB values if one carries many small particles and the other a few large ones.
This is why ApoB and LDL-C sometimes disagree, most often in people with insulin resistance, elevated triglycerides, or metabolic syndrome, where particles tend to be small and numerous. In those situations ApoB is associated with risk more consistently than LDL-C, and several cardiology guidelines now list it as a preferred or complementary marker.
ApoB responds to the same levers as LDL cholesterol, dietary pattern, body composition, and lipid-lowering medication, so it works well as a tracking marker. If your ApoB is above roughly 90 mg/dL, or if it stays elevated while your LDL-C looks fine, that pattern is worth discussing with your doctor.
