Free T3 (pg/mL)
Free T3 measures the unbound, biologically active form of triiodothyronine, the thyroid hormone that actually drives metabolism at the cellular level. People test it alongside TSH and Free T4 to see whether the body is successfully converting stored thyroid hormone into its active form.

The importance of Free T3
The thyroid mostly secretes T4, a storage form; the body then converts T4 into T3, which binds receptors in nearly every tissue and sets metabolic pace. Free T3 measures the sliver of T3 not bound to carrier proteins, the fraction available to do work, which is why it is often described as the most downstream reading of thyroid status.
Conversion of T4 to T3 is itself regulated. Prolonged calorie restriction, significant illness, high stress, and selenium or zinc shortfalls are each associated with reduced conversion, so Free T3 can drift low while TSH and Free T4 look normal. In hyperthyroidism the opposite pattern appears: Free T3 often rises first and furthest.
Because Free T3 fluctuates more than the other thyroid markers and assays vary between labs, it is interpreted as part of the TSH–Free T4–Free T3 trio rather than alone. A low-normal Free T3 with symptoms, or any clearly out-of-range value, is worth reviewing with your doctor in that fuller context.

Yes, sustained large calorie deficits and very-low-carbohydrate diets are associated with lower Free T3, which appears to be part of the body’s adaptation to reduced energy intake. Levels generally recover when energy intake normalizes; persistent low values deserve a broader look.
